What you get: monthly prescription renewal, access to WHHW provider for two, 25-minute phone calls/month, access via email with a 24-hour turnaround to address any issues or questions you may have, or one 30-minute telehealth visit/month to discuss anything related to your treatment plan, medication questions, access to learning modules via WHHW website.
Initial telehealth consultation visit: 90 minutes

If you prefer a home visit: 90 minutes
Cost: $300 within 50-mile radius of Broken Arrow, OK 74012. If this is your option, the follow-up visits can be via telehealth.
Both visits include: Full history: medical history/surgical history/social history, medication review, and interview. Discuss symptoms. Review realistic health goals, short and long term. Develop and decide upon treatment plan possibilities. Evidence-based diet/lifestyle management recommendations. Determine whether there is a need for a specialty referral: Cardiology, OB-Gyn, Pelvic floor physical therapy, etc. Weight and vital signs: heart rate, blood pressure, respiratory rate. Time will be spent discussing FDA-approved treatments for your set of symptoms, education will be shared regarding research that supports your journey to wellness, finalize wellness plan together.
3-week follow up, telehealth: 30-45 minutes
Cost: $150.00 Discuss any potential side effects, questions regarding medication therapies, any changes in symptoms? Progress towards goals? Set-backs? Determine whether there need to be any changes to your treatment plan.
6-week follow up, via telehealth: 30-45 minutes
Cost: $150.00 Weight and vital signs. Review plan of care and any questions?
Visit to address any questions regarding your treatment plan between visits can be done via telephone or email for non-emergent issues. These typically take 15-20 minutes and can be performed without charge.
Cost: $50.00 if there is a change to the treatment plan previously established.
If you feel you need more time, please schedule via the website for the next available appointment time.
Cost: $100.00 and we can spend at least 30-45 minutes addressing your concerns.
Six-month follow-up: 30-45 minutes
Cost: $100 This visit will cover any questions/concerns you may have. Check-in on progress toward your goals and address any issues you may be experiencing.
